I have not done this myself but, I don’t see why you could not Autoclave them for sterile conditions

Do these clean dirt under nails?

Yes, if you use the end that doesn’t look like a “spade”. It tucks with the more pointy end neatly under your nail and using gently will remove any gunge under your nail.
