Q: 1) what is the dimension of the platform/basket? 2) will this fit a 650b city bike with disc brakes?

The top surface dimensions are 12 x 14 inches. It should fit your bike without issue, but I cannot promise that it will, because I used it for a 700c. I would be confident that it would work, however, and it works really well. I hope this helps. :)

Q: I am trying to mount this on the fork eyelets instead of the wheel axle. Can this be done? If so will it support the same amount of weight?

yes i did with some appropriately size bolts and oversized washers regular washers and some shims i used some nuts as shims and drilled them out for the bolts not hard to figure out works great lock washers too

Q: Can I mount a milk crate on this rack to carry my dog?

Yes you could but it would be heavy. The rack is very well made and I have a dog basket that I carry my 10 lb dog in. The rack keeps the basket from rubbing on the tire. It does make it hard to control the bike at times, especially if my dog moves around. If your dog is over 10lbs I would use a back rack
