I have not purchased this item in a while, but yes, the last time I ordered one the chip was included.

Yes it read them with no issues

Q: Has anyone experienced any issues with the cartridges leaking?

That is not a problem that I have had but that doesn't mean it cannot occur. I use about 3 - 4 cartridges a year.
