Is this comfortable and snug enough to wear while running?

Yes. There are adjustment straps on the sides to set the vest as snug as you like. I have used my vest for over a year for running, exercising, and boxing bag training.  Very comfortable, durable, and has enhanced my workouts.

can i use the push up vest with a bad lower back

Hi Curtis, I also happen to be a CPT AND I had lower back surgery in 2010. In answer to your question you can use the vest for push ups because most of the weights rests on your upper back, just make sure you have good form and if you incur any aggravation to your lower back you may want to use a support belt similar to the one people use when lifting. I have one I use when I do lower back extensions as well as decline push ups. One back surgery was enough for me. I'm 56 and don't want to risk further problems.

I am 5/6 and 144 pounds. I'm a runner, spinner and weightlifter. Will 20 pounds be too much?

I think this is a personal question. If you have neck and back problems even as a weightlifter I would recommend starting at 12 lb and moving your way up. My boyfriend is a serious weightlifter and he is 6 foot  2  and 220 lb and built very nicely and even he started with the 12 pound. And this is coming from a guy who deadlifted 475 on a regular basis. He used the 12 pound for about 4 months and is moving up to the 20 lb now. And in four months he will move up to the 30. I would definitely recommend wearing a t-shirt under it.

Is the 20lb low profile enough to not be very noticable under a shirt?

It is as low-profile and snug fitting of any weight vest I have ever used.  But it is not so slim that you wouldn't notice it under and t-shirt.
