is this ground up powder from the pepper or was the pepper smoked or cooked in any way that might change the flavor?

The pepper isn't smoked. It is simply dried and then crushed. Full flavor of the pepper is retained this way.

Why is this not very hot?

It's hot enough to get the job done. But if you're expecting raw Carolina Reaper heat you may have your expectations too high, that's what I did. For anyone who likes spicy food it's fine. If you love serious heat and you're looking for intense spice be prepared to use a lot. I made hot wings twice, about 15 each time, and used nearly the entire thing between the two meals. They were certainly hot with a few heavy sprinklings but nowhere near what I wanted. By the time they were as hot as I was shooting for I had used nearly half the bottle, in addition to several other spices.
