So is this just to add a sub amp or can it work on a 4 channel for mids and highs also?

Yes, you need an rca splitter for the 4 channel from the main output and run the sub alone from the sub output. The main and sub are left and right only ( no front and rear) that's why if you want to run a 4 channel amp you need that splitter.
