She designed her own for herself, and wanted to help others track their health. These books are great to log health and eating, as well as for tracking down allergies with delayed reactions and adverse food, pollen and other allergies, so you can avoid problems, improve your diet, balance imbalances, keep track of your weight, carbohydrates, fats, proteins, sugars, etc. A great resource to fill in and share with your doctor, nutritionist, NP, etc. Extremely helpful in problem solving, or tracking what works for you in your diet, weather, exercise, and so on. Everyone should have one ofthese! My old doctor was impressed with one of mine and he ordered cases of them for all his patients. He told me he used them for charting info for later research, too. These books are VERY nice to have around. Review: Perfect for keeping track of your health - This is my second HealthMinder Personal Wellness Journal. I have a number of chronic health conditions and I'm appealing a denial for disability, so it helps to keep daily track of my symptoms, location and type of pain, medications taken, vitamins and supplements taken, food and drink ingested, and so forth to hopefully help myself, my doctors, and my attorney. I really like this journal. Part One - Getting Started has an introduction, tips for using your HealthMinder, and symptoms glossary. Part Two - Daily Record of Conditions & Habits has journal pages for 95 days. Part Three - Additional Health Records has personal medical history, medical tests, insurance and pharmacies, healthcare providers, purchase record, notes and questions, reference calendar, and other journals and reorder form. At the end of the journal is a vinyl utility pocket that you can put records or whatever in. This is a very well-thought out, organized, and thorough journal. In the daily record of conditions and habits, each day is spread over two pages. The first page has a space for the date, day, today's weather (hi/low/and conditions;) AM/PM for: weight, temperature, blood pressure, and sugar level. Also included is: hours slept last night (number of hours, sound or restless;) naps taken today (how many, total hours;) drugs/medications (AM/PM, quantity, description, strength;) vitamins/herbs (AM/PM, quantity, description, strength;) physical activity (activity, hours/min;) pain/discomfort/skin changes to be marked on a figure of a man front and back (I put x's where I have pain and symptoms on the figure;) a pain scale to note next to the figure of where you marked pain and symptoms (1-2 mild, 3-5 moderate, 6-8 intense, 9-10 severe;) and In general today I felt: good/fair/poor. The second page has today's conditions and symptoms where you explain your conditions or symptoms in the following areas: ears/eyes/nose, mouth/throat, head/neck/back, shoulders/arms/hands, chest/heart, respiratory system, digestive system, hips/legs/feet, male/female organs, skin, mood, and other. There's also a section that you record what you eat, drink, and anything else you monitor intake of (fat, calories, sodium, sugar, protein, etc;) breakfast, lunch, dinner, and snacks, and you can total up whatever it is you are monitoring in column A and column B. There is also a comments section for you to mention anything else. I get good use out of daily monitoring my health, and I am so glad I found this journal to make it easy. I thought I was just going to have to write stuff down in a notebook, but this has everything that I need in an easy to use and easy to see (for tracking) format. Comprehensive for tracking my rheumatoid arthritis
Overall this is a great, comprehensive journal. This journal has roughly 76 days worth of pages for tracking (manually counted). The back contains pages for medical history, tests, insurance, providers, purchases and notes. I searched high and low and this was the only journal I located on Amazon that was the closest to meeting my needs. It allows me to track my physical status, meds, activity, pains, symptoms and nutrition. This way I can better visualize any trends or changes that may relate to changes in my meds, activity level or diet. It helps remind me of things to discuss with my doctor. The body diagram helps with quickly noting and referencing pain locations and levels. I rated with 4 stars because the area to make additional notes each day is very small. If you want to go into detail on a topic or note something not covered, it's very limited.

Keeping track of our health
I ordered this for my wife, and it is as advertised. I will be ordering one for myself. I don't know why we have never thought about keeping a personal medical journal. I have shared this with my friends and many of them are ordering one for themselves.

Excellent Health Journal
I’ve been searching for a journal that I could write in daily to potentially find patterns of what prompts flares or triggers bad days. This journal has room to include foods for Breakfast, Lunch, Dinner & Snacks . Additionally, I can keep track of activities, the weather and of course how my body is feeling. There are front & back body pictures, much like a doctor has chronic pain patients fill out. My favorite part though is the breakdown of where I can write specifically what’s hurting & give descriptions. Taking the journal to the Dietician’s office, we were able to discuss changes and she could see the food journaling. Physical Therapist is able to monitor flare-ups and modify based on my writing since foggy brain keeps my memories stunted. I can hand him the journal instead of wracking my brain and coming up empty. The most difficult part is remembering to write in it daily. It would be awesome if this journal had an App to accompany it that allowed for tracking calories, carbs, etc. for the foods, some sort of chart or graphs to see flares, types of pains in afflicted body areas, etc. A way to email the details to my providers as progress notes. If I noticed an obvious pattern, I could email positive proof and request supplement changes, modified diet or even physical therapy. For now, I’m happy to have a journal that meets my needs. Definitely recommend!

Have had this journal in the past and keep on buying it because it is probably the best on the market. The ease of use coupled with all applicable info makes this a ready reference for anyone.

This is exactly what I was looking for and I could not be happier. I have tried using various phone apps to track my health, and found it cumbersome and of limited use. This journal, however, meets my needs perfectly. On a two page spread for each day I can easily record things I am tracking (such as weight, blood sugar, blood pressure etc), what medications I take that day (including strength and how many taken), a summary of physical activity, the food I have eaten and most importantly my symptoms IN DETAIL! This includes a little diagram of the human body when I can mark pain areas, intensity and time of pain. As a person with several long term conditions I am finding it invaluable, particularly for helping me track things like what impact a change in diet, medication or exercise has on my symptoms. I would totally recommend this to anyone wanting something to help track their health. Note: if your prime interest is in monitoring your diet and exercise, then this may not be the right choice, since space for those things is limited. This is primarily a symptom tracker.

Not enough room to write things - too crammed with too many little boxes and things to check off. They do cover everything you might want to record, but it's just too "busy" and too much info for me. Used it a week and tossed it.

I bought this as I have been ill for some time with Fibromyalgia and have to constantly see my Gp. This journal is a godsend as it gives me places to write down how I am from day to day so that I can report back to her accurately, which without this is hard to do as remembering how you have been from one day to the next over a given period is hard enough for anyone! I would definately recommend this to anyone who is undergoing treatment or is worried about a health concern as it helps to keep an accurate record easily and for you to show your medical advisor without having to worry about remembering details from dates/times and symptoms.

Great that this is a spiral bound book. Pages are very well laid out with plenty of detail for you to complete and fill in. Some things I would not bother with everyday, but it's a great health checker.
