Is this just like, a wide piece of wood with slots in it, or does it have cushions for your lap?

There is no cushion on underside, just wood

What are the dimensions of the mouse surface?

The length of the board is stated on the page. You can use excel to measure the pixel length of the board from a screen shot by using 10 wide cells. From that you can figure out the length of the board in excel, stretch another cell to be as wide as the mouse pad. Now you know the width of the mouse pad and board in excel. Then using the total board length in inches you can extrapolate the length of the mouse pad. This is basic problem solving, not MacGyver level improvisation.

How wide are the slits for the phone and tablet?

Wide enough for phones. Contrary to the picture, its not wide enough for tablets (unless you have a very small ipad).

doesn't this make your legs go numb after awhile?

I use the lapdesk for a keyboard and mouse (as opposed to a laptop). I haven't had any issues with it resting on my legs although better stability can be had by resting the left and right sides on the arms of a chair or a pillow under each side if on the couch for support.
