Can this be used for after surgery to protect the wear while it heals

Yes. I used them to be able to wash my hair post surgery on my ear. I did also use tape to secure the front edge to ensure no water got it. Never had a problem with any water getting in.

Is this suitable for 4 month baby?

I have never tried them on a baby but they are just small versions of a shower cap that can fit around any size or shape ear and I find them useful. However they are very small and can slip off easily and might end up in the babies mouth so they would need to be supervised at all times and not be left alone with them on.
