There is 2 ir sensors and a 3.5mm jack connector.

Q: Can this be used to connect directly to an XBox one's IR port to control TV/Cable/STB?

Yes it can that is what I've used it for. It is a single jack into the back of the Xbox one. I then have one emitter on the to and one of the set top box.

Q: My box is behind the wall and my tv is infront how do i use it

You need to use it with an infrared repeater like the Marmitek PowerMid XL infrared extender set. The iR end of the cable is taped over the is receiver of your box which the other end plugged into the receiver part of the extender.
