After moving the limbs a few times the balls unscrew themselves, loosen & the limbs swing everywhere by themselves. How to prevent this?

You need to use the special glue loctite 638 glue. Glue the balls to the threads.

No, it does require assembly as it is a bag of nuts and bolts. But has all the tools included in the box that you'd need to assemble it.
