So this looks like it only fits in a spare tire without a Rim?!? Am I correct

This subwoofer fits on top of the spare tire, rim included. It should fit perfectly on a donut sized spare.

Do I need an amp install kit? Or put another way, do i need a separate amp and power turn on / ground wire?

This subwoofer has a built-in amplifier. You will have to run a power and ground wire. You will also need to attach the rca cables or speaker cables to the unit for it to turn on and off. You will be supplied with all the necessary wires included in the kit. Follow the installation manual for your particular set up. It is a great sounding unit.
