I looked at my outlets but dont see any grounding hole cna anybody explain this?

It’s the mouth of the smiley face made by the outlet….( D= ) < that’s the outlet, the ground plug it’s the “D”

Do you put the cord into the adapter and plug adapter in the wall outlet or put plug cord directly in round wall socket?

You put the tip of the cord plug into the little round grounding hole of an outlet.

Just tested mat with a grounding tester and it is NOT grounded ?

It may not show on a meter. It’s called a soft ground
