the package and manual are japanese. the cards - as all UNO cards - dont have any language. The UNO rules in Japan are same as in Europe. If you know how to play UNO, then you can play it also with this cards

The card are the same as other types of Mattel uno card but yes the instructions are in Japanese but u can find the English version online very easily

Can you use these cards in the uno extreme machine? Is it the same game but just with Mario etc on the cards?

I bought it for a friend. Don't see why you couldn't use it in the extreme machine though!
