Yes, so long as your 3DS' system settings is set to the Italian language it will do Italian subtitles. (I'm not at a certain point in the game to activate a cut-scene with voice acting to confirm if they speak Italian or not, but as for subtitles: yes)

Q: Can this be played as a stand alone or do you need to know the story?

No prior knowledge of Dragon Quest is required to play any entry in the series, every game is a self-contained story. There are some loose connections regarding the world it is set in, but I promise you it is not required at all to understand the story. In fact, Dragon Quest 8 is probably the best starting point, and if you enjoy it, you can do what I did and go back and play the older games (they are all fantastic).

Very late but for anyone else wondering the same thing, I played this game when I was 9 and replayed it when I was 18. I never noticed anything rude (sexual) other than the "Puff-Puff" joke, but you'd have to check that out yourself, and even then, you have to talk to an certain NPC out of Hundreds, and Hundreds on NPC's in one of the final acts. This game may be difficult for a child. Even at 18 I found myself constantly thinking, constantly trying to figure out how to proceed and earn XP correctly. For a young child it may be very difficult and they may get bored because of this. It depends on the child. If they love brain teasers and hard work, otherwise you may be better off getting them a more generic title in case they just give up as its too hard for them to figure out.
