Q: how comfortable are these for very narrow ear canals (I can only get comfortable with the smallest (green) Howard Leight foam plugs)?

I found these very small.You need to be careful not to push them in too far!

Q: Do these make music sound garbage?

Not garbage just quieter. Takes out some of the base as well.

Q: Do they also protect your ears from engine noise, when driving a race car? Or just wind noise on a motorbike?

Hi! Good question. It will also filter out excessive engine noise. The filters are geared towards the harmful frequencies and a proper race car can definitely hit those tones.
