I bought it for my grandchild last summer. It is USCG approved.

Q: Do it hold them up in the water without sinking?

It kept my son floating, so as long as you stick to the weight restrictions it should.

Q: Will the infant one keep my 16 month old afloat straight up or will it make him go on his back? He is 26 lb

The purpose of the large head guard is to keep the infants head above water . If the baby can hold his head up and push his feet down then he will be vertices in the water other wise he will be floating as if laying on a pillow above the water.
