Can you get these slippers in a larger size?

Yes! Our latest model, Nov 2022 and on, is larger, 11.8 inches long, so should fit most feet. It should fit up to a size 11 men. Hope this helps!

Do they heat the bottoms of feet, or just the top

Yes mine do heat up on the bottom of my feet.

What are the instructions for heating?

Not sure if this will answer your question or not but I got them for my wife and she usually puts them in the microwave for 30 seconds .Move or less depends on what is comfortable for you,just do not over heat them.
