Q: Will it securely fit an ipad pro 12.9 with a otterbox defender case (otterbox defender case 12.9 dimensions l x w x h, 10.4 x 7.8 x 13.1 inches) ?

First let me say that this is a well made product - Ineeded the unit to hold my iPad Pro 12.9 with a Otter Box -Defender case. --There are a few comments stating that the unit will hold a 12.9 Ipad Pro with a possibility it would hold the Otter Box Defender. The truth is that IT WILL NOT HOLD a Otter Box Defender Case. The clamps come up about a inch shorter than they should be So for my purpose it does not work. IF you just have the iPad Pro 12.9 without any protective casing the unit works fine .

In portrait mode yes but not landscape; It’s designed specifically for iPads

Q: when in the highest position possible, what is the distance between the table and the bottom of the tablet?

11.75" if the iPad is vertical (perpendicular to the table)

Q: Can you flip the stand where the iPad could face down?

Yes the IPad can be positioned any direction you wish.
