Can this be used to slowly increase the voltage in a power wheels type car? Wanting to increase voltage over time from 12-24v?

You can increase the average voltage overtime. Basically you need a 24V supply. And then you can pwm from 50% to 100% of whatever the range is.

The manufacturer says it needs, requires, must have a heat sink. so why did anyone manufacture it without a component that it absolutely needs???

I have used this board with and without heatsinks to drive NEMA 23 stepper motors at 1.5-2A, and haven't any significant heating in either case.

How do i use digital high & low with no pmw, i left ena disconnected no luck. help!

Connect to the PWM pin and drive it high for max speed. Low for no rotation

Can you recommend a pwm motor controller board for 12v x 10amp motor with an input signal from 0 to 55 mv with 100% duty cycle at 25 mv?

0-55mV? Thats not even considered a signal by most any controller for PWM.
