Is it normal for this to taste bitter? Thanks

Yes Neem herb tastes bitter. But neem good for health.

Do you use this instead of toothpaste, or in addition to?

I use this once a week instead of toothpaste. I don't use them the same time

Can I mix this with toothpaste?

I don't think you would want to. This is in place of your toothpaste. This product along with the drops has helped me and my moms gums tremendously. The paste makes my teeth feel so extra clean.
