The label says "manufactured for fitlane nutrition" so who is the manufacturer "for" fitlane? why isn't there 100% transparency on the manufacturer?

Fitlane has NOT disclosed the country of origin for their allulose product. When I encounter this kind of shading in food product listings, I assume "China", and accordingly, I assume China in this case. One reason China sourcing is frequently hidden by USA food distributors is the widespread contamination by lead, cadmium, and other heavy metals over virtually all important Chinese agricultural areas. When contaminated products are sold in California, a "Prop 65" warning is required on the packaging...and I've seen this on competing products. There's no Prop 65 warning mentioned by Fitlane here, but this may mean only they don't sell in California. We simply won't know where this stuff is from until and unless Fitlane tells us.

What is this product made from?

Unlike what W.M.T stated it is not in the description. The description says “ Allulose is a rare sugar that occurs naturally in raisins, figs, and other foods”. It fails to say that allulose is typically derived from corn, because it’s cheap and easy to get. Unfortunately though they don’t ever actually  include what they got the allulose from just that it’s allulose. I’ve tried countless times to find allulose made from dates or something else because I can’t have the corn allulose. Good luck!

What country is this product manufactured in?

The label does not say specifically, but it does say the product was made for a US based company and a USA flag is included. So, it certainly implies it is made in the USA. 
Country of origin rules have changed. It used to be that if a product was imported, the country had to be disclosed on the product. If no country was mentioned, we could assume it was made in the USA. I understand that now we have treaties that prohibit country of origin labeling for some products to hide the fact that some products are exported. I am opposed to such terms.

Is this granulated or powdered? The description keeps referring to it as both powder and granulated. It’s only one or the other.

It sounds strange but it’s in between. Its not as course as granulated, but isn’t as fine as powdered.
