Q: Does this work only when dog is at your side in heel position? When we walk casually (not heeling) it's usually between his legs and under his belly

It works whether dog is in front, side, or behind. When they're in front make sure the leash is not between their legs. My Staffy/Boxer usually walks a little ahead, and to the side of me, and it works great with him. You can also use the leash clip to clip the collar to the harness at the chest, that might help. I use to have to do that, but only while he was getting used to it.
