the question doesn't indicate which product but my straps stay tight on the Ruffwear switchbak.

I wouldn't suggest this style of harness for a pomeranian. You'll need something that comes in an extra small and isn't as bulky as this one.

Q: What size should I get my 20 pound Boston Terrier?

Its could be an XS but please go on Amazon and look up the switchbak product. They have a sizing chart and a diagram which will tell you what measurements to make to determine the size. My 34 pound golden doodle uses a small.

Q: Anyone put ice packs in the pockets to keep them cool? Or is the padding too thick?

I haven't tried that. I bought the cooling thing that you can attach to it and I usually put it in the fridge or cooler if we are bringing one.
