Yes, there lines for the title and author. We use these in our church library and I am pleased with the quality.

What is the actual card size?

The card size is 3 inches by 5 inches and the card is quite stiff so easier to put in and take out. This set of card and pocket holder is the best. I've bought other cards for the same price or more that are very flimsy and don't have a column for the date. Buy these. You will not be disappointed.

Is it possible to remove these envelopes without damaging the book?

As long as the envelopes still have the adhesive liner paper attached to the adhesive, the envelopes should still be easy to store and move around. If the adhesive liner paper has already been removed and the envelopes are already attached to the inside of books or other media, probably not. They’re the kind of standard library card envelopes found in library books.
