does it work on hard wood floors with old urine dark stains

I would not use this product on hardwood floors. There is another product for hardwood.

does the work on a bed mattress?

Since the mattress can't be washed it will retain the scent of the spray. But it does break down the urine and excrement. If you have a Spotlifter type of cleaner I would use that too
