Will this fit over a heavy duty extension cord?

Yes. I have a thick yellow cord from a heavy duty outlet strip going though it.

How do you hold the cover down to hard floor?

Many thanks for your interest in D-Line Floor Cord Cover, they are made from a flexible material which simply uncoils to lay flat (with no need for adhesive tapes). What is it made out of?

This floor cable protector is made in the USA from a highly flexible, abrasion resistant PVC that effectively uncoils to lay flat.

Does this lay nicely on carpet?

Yes, it has some weight to it and will lay nicely on carpet, tile or hardwood.
