does this come with the Belt sheath

Yes. It comes with a hard plastic belt holder that secures the scissors securely. And the holder fits tightly to the belt so it does not slip off.

how do I get my name added to a blade ?

You have to order directly from Leatherman for laser etching. Their website has a customize option where you can design patterns, drawings, different fonts for names, even upload your own photo.
