What's the difference between this and the regular OptiWand?

The length of the tube is longer and the full cup end is slightly wider. Both function the same. Both offer money back if unable to use

What is the long part sticking up on one of the bases?

The thin part sticking out is kind of a reminder that the full side is for regular lenses, large fingers and long fingernails . The opposite side eith a split is for small lenses or eyes .

Can it stand on the countertop ?

Yes you can stand it up on a countertop. If using sclera lenses you stand it up, place lens, add fluid then lower down to insert

can u use this for hard contacts?

I don't really know; this device is mainly for soft contacts. You might be able to use it for getting hard lenses out of your eye--but, it's not particularly designed for hard lense removal from the eye. Frankly, its difficult to use on soft lenses, in my opinion.
