I am not sure , if I had to guess it would be a girls 11 or 12, I purchased this when she was ten and it still fits her with room to grow, she is eleven now. I hope this I was able to help.

Where can I find the headpiece that goes with this kimono costume ?

There is a headpiece that comes with the costume. However, it's pretty flimsy as it does not hold up/stand up very well. I took the liberty and made my own. Hope this helps.

What size is toddler size?

I got a medium for my granddaughter, she's 5 yo, 45 pounds. It's a bit small. I would imagine the toddler size to be 2-3T
