How big can the vegetable be to fit?

I am sort of guessing, but I think at least 1 1/2 inches in diameter, possible 2. I love this thing. Compact easy to store and has two different sizes of noodles. You just flip the devise over for the second size. I highly recommend it.

Is this something that you manually twist? If so is it easy to twist and cut?

Yes, It depends on the firmness of your vegetable, it looks like sharpening a pencil, I use a little twisting pressure to cut. The blade is so sharp that I cut it quickly and finally I was so surprised to get a long carrot stick that embellish my salad. Nice product.
