Yes it comes with hardware, and instructoions. It's pretty easy to install. 15-20 minutes at most.

What's the diameter size of that bore? & anyone have an idea how high it raises the CAI ?

Not sure of bore size but I ordered it for my 2008 f150 and it did not fit with my CAI so I sent it back.
