They have the standard anti-tamper cap. When you look at it you can tell it it’s been open. Afterwards it’s a screw sealed cap

No, you don’t have to refrigerate it.

Q: Does this actually contain lavender at all? What exactly are the “natural flavors”?

No either one of the bottles I ordered had a seal on them.
