Does the tank have any protection in the event someone attempts to drill a hole and attach a towel rack etc to the other side of the wall?

Not that I remember seeing. The frame around the tank is nice and sturdy but I remember the tank being somewhat open. We love ours. It was very difficult finding a plumber who could take on the challenge but once we did and got it installed, we were very happy with it. Because we were working with in such a small space, we had to pack a lot behind that wall (there were electrical wires and outlet boxes, etc. as well as The tank and pipes for the sink) so I would not even attempt to hang anything above my toilet! (Other than something that could stick on the wall.) Anyway, Good luck!

Whats the drill size to fit a drain pipe on a 2x4 frame, without expanding the 2x4 wall frame??

3-1/2" hole saw. That skims the edges of the 2x4 plate. I ran a guard strip on both sides just in case, even though I did the work and was aware when screwing in the sheetrock.

What size are those 2 rods ?

It's not clear what you mean by '2 rods', but here is the specification sheet for this carrier - https://www.geberitnorthamerica.com/local-media/spec-sheets/gna7225-111-798-00-1-spec.pdf

Can the geberit 366.914.16.1connector w/offset for rh horizontal waste fittings be used on this frame to drain to the side in a 2x6 wall??

It can go right, left or straight down if you choose!
