The pillow is highly collapsible. I have a small wrist also. Fit should not be an issue.

Will a 44mm Panerai fit in here with an XL strap?

I have tried large watches with both straps and steel bracelets with no problems to date. You should have no problem with your watch. This winder works very well, very good quality for the money.

This is pre-programmed for 900 turns per day. My timepiece requires 650 turns per day. Can this winder be programmed for 650 rather than 900?

I've had this winder for about 1 1/2 years and it works great for my wife's Rolex. You can't specify the number of turns per day with this winder; however, there are 2 settings - 1 for 24 hour movement and another for 12 hour movement.
