White, Black, Purple,Blue,Red, and the other one has already been used and i don't remember what color it was.

Will a pen fit in the spiral?

Yes, it will - even Sharpie size markers will fit

Is one notebook small enough to fit inside a scrub top pocket?

I don't know the dimensions of a scrub top pocket, but I would (from memory) think it would be too wide. They have a slightly smaller notebook (5 1/2" x 4") that may work. I would suggest the 5 x 3" notebook. They have both a spiral notebook (spiral wire on the side) and a 5 x 3" spiral notepad (spiral at the top). The notepad would probably be easiest with the spiral being at the top wouldn't fight on the way in/out of the pocket.

Does this have a clear plastic pocket on the inside?

It has a pocket, but it is not clear plastic. There are two pockets on front and back of the heavy first inside page. These are my favorite notebooks - very study and well made.
