What is the difference between this product and obagi medical clenziderm m.d. system ? i'm 33 years old with moderate acne skin

Number 3 is a cleaner called Skin Brightening Cream and Number 4 is cream for exfoliating cream. You use 3 and 4 in the morning.

Do I need to rinse this off after applying?

You dont rinse off, this stays on your face, just like 3, and the sunscreen
