How would you make smoothies out of this?

Here is how I make my smoothies; I also use Pure Protein 100% Natural Whey Protein Frosty Chocolate powder, Recipe: 8 oz cold water, 2 ice cubes, 1 frozen banana, 1 scoop measure of Frosty Chocolate powder, 1 rounded Tbsp of PB2, using the blender on 'ice crush' until frozen fruit and ice is crushed, I add either frozen strawberries next or frozen peaches and run on 'ice crush' then on the 'mix' setting of my blender until it is smooth. Sometimes I will use Vanilla Almond milk in place of water, I may or may not add Kale to this. My husband and I will share this for desert as we are dieting and this makes a refreshing cold alternative to a high calorie desert.

I got the 'brick' package and was unable to figure out how to open it. How do you open the brick?

Just use scissors to cut down far enough to release the vacuum and create an opening wide enough to pour the contents into a lidded container. This will allow easy access and help maintain freshness of the product. You could also transfer the PB2 to a ziplock-type bag, but I personally find this more cumbersome for handling and storage.

How much PB2 do you need to make a cup?

That's a great question that I wouldn't mind the answer to, however I have just 'eye balled' it in a bowl for use in recipes. I will end up with a little more than I needed, but I just scope the leftover into a small container and use it later for another use. It keeps, and it's already made. (Like for dipping pretzels into for a snack!)
