Is the bottom of this padded so that it won't scratch a glass table top?

There is no bottom to this. It slides right over the tissue box and is open on the bottom. What touches the tabletop is the bottom of the tissue box and the edges of the cover which are not padded. The metal edges are smooth and I can't imagine that they would scratch a table top.

If you pivk it up will the tissue box fall out? Or does it fot snug?

The tissue box will fall out. I bought this metal box because of the weight. When I am pulling a tissue up out of the box, especially when the tissues are almost gone, the metal box is heavy enough to allow me to pull tissues out, without pulling everything up, tissues and metal box. Because the bottom of the metal box is open, it is easy for me to put a new box of tissues in.

Is this real pewter or only pewter color?

I do not know that answer. However, I bought the copper one and it's beautiful and appears to be made of copper. No paint or anything odd, simply lovely craftsmanship. Hope this helps.
