Do both tvs have to be on for the signal to go through?

Hi Will, Regarding of HDMI 2.0 Splitter 1x2 mini , both TVs have to be on for the signal to go through. Thank you for attention.

Can i use this to connect an apple tv 4k to a dolby atmos soundbar and also a sharp 4k hdr television?

Dear Customer, HDMI Splitter 1x2 4K 60Hz 4:4:4 supports your application well. Please set splitter EDID to 4K, Connect 4K TV to Splitter OUT 1. EDID copy OUT 1 prior. Connect Soundbar to OUT 2. When Apple TV play 4K HDR Dolby Vision,HDR VRR, please do NOT turn on SCALER, due to splitter does not support down-scale 4K HDR Dolby Vision,HDR VRR to 1080p display. Thank you for attention.

Is this only for dolby atmos? will it work for dts 7.1 or 5.1? what if i do not want atmos?

Dear Customer, HDMI Splitter 1x2 mini supports Dolby Digital 5.1/ 7.1 ch audio well, also Dolby Atmos pass through. Please ensure HDMI display devices / soundbar / TV supports 5.1/7.1ch audio in advance, set EDID to 4K or COPY mode. The hdmi splitter can pass through audio signal . Thank you for attention.
