Does this have an opening for oil changes or do you need to remove skid plate?

Yes, it does but would create a mess if I attempted an oil change with the skid plates still installed on my Gen 5 (2018) 4Runner. Additionally, in order to change the oil filter, the front skid plate must be removed. Such is the case with the factory skid plate.

Is there any access for basic service like oil changes or transmission fluid, or does the whole plate need to be removed for any service done?

Down side is that these plates don't have access ports so you have to completely remove the front plate which is just below the engine. Not too hard only 5 bolts. And you have access to the hard to reach oil filter.

Fits 2022 4runner with kdss?

Yes, I believe it would fit your 2022. I installed the plates on my 2018 TRD with KDSS. ARB is great stuff (I believe the best aftermarket option) but was disappointed that I lost a good half inch (or more) of ground clearance over the stock plates. The KDSS option which I love, already has a lower ground clearance than the other suspension packages. Additionally, I picked up some wind noise with the change. While I had destroyed the stock front plate (twice) and needed to replace it, I do question the installation of aftermarket plates even as good as the ARB's are. Maybe I just need to be more careful when offroad?

Does this include the whole skid or just the front only?

The whole underside to the trans, including a skid for the transfer case.
