Does this have the threaded shaft built in?

No; it has no shaft. I had to return the castor assembly for that reason.

what is the size of the wheel ?

5 inch diameter, as stated. They worked 100% fine on an Invacare Reliant lift. The small dual caster is 3 inch diameter. I could not find them on Amazon so I got them at Southwest Medical. They come one to a package for $47 plus $19 shipping, Part#: 1123560 - Caster, Swivel Dual (3"). I wish everyone was more clear that they come one to a package as I want 2 of each for replacement since they tend to wear out at the same time. It is not too bad to replace if you have the right tools. And maybe some WD-40. A big open end wrench (I think 3/4 inch) and a vice grip to provide extra torque. Getting the old, stuck ones off is the hard part since there is little space to work. Installing the new ones is easy.

Is this a set of 2 castors or just one?

It is NOT a set of two. I ordered two. And cost me $86.14.
