Q: Can it be used in shower?

This is a battery-operated device. It's definitely NOT water-proof.

Q: Can you buy just the suction cup? I have this in the mirror is fine but the suction cup won’t stick to the mirror anymore.

The same thing happened to me. Very disappointing. I don’t know if you can order the suction cup.

Q: Can you use it without the flexible extension? ie) Attach mirror directly to suction cup.

Yes! There are other brands (and maybe this brand) available without the extension. I've bought a few in the past that attach directly to mirror. Do an amazon search to find the ones that are in stock now. Good luck.
