Will this fit with a macbook pro mid 2012? And should it work with the samsung evo series ssd?

NO This does not fit Mid 2012 Macbook Pro systems (9,2) models

I have an early 2011 Macbook pro 15", will it fit ? Also how do i find my MBP's MB*** number ?

It does fit, but the back of it touches some metal in the computer and shuts it down, I had to take it off. I recommend you look for another brand.

Does anyone know what the little white switch on the side of the drive caddy is for? And how should I set it?

If I recall correctly. It's a slave/master switch. I'd say leave it.
