Q: The website states small or large what size is a small insert on average i am a size 5.5

I'm a 5.5-6 and bought small which fit fine.

I don't think this is suitable for open-back shoes/sandals. You better get something similar for that will prevent your feet from slippering.

No, the heel cup doesn't not make shoes smaller , because it's edges are not thick.

Q: What size for 6-7 shoe. Is it better to go for small suze or large?

I really don't know for sure but I bought large for my size 9 and they were too small so I'm guessing a large might be ok for 6-7.
