Does anyone know if there are plastic card holders that would fit this binder?

Yes here is a link Filofax Business Card Holder (B343616) https://www.amazon.com/dp/B004YWGPO0/ref=cm_sw_r_cp_api_i_0aHOCbWE1EHW2

What size log pages does it fit?

The same size as the PADI log book.

Does this come with blank log pages?

Yes, it did. Not all log books are created equal though. Some have more/less information, but it never hurts too over document. You can always put other sorts of log pages if it doesn't suit you. I love mine though and have received many compliments over the few years I've had it.

How many pages will it hold or how large is it?

this book will hold a bunch of log pages! maybe around 150+
