Q: is there a finish on the wood or is it natural?

It isn’t natural, has a varnished finish.

Q: Is the seat flat or slanted?

The seat is flat and so is the bottom one

Q: Is it recommended to apply Teak oil prior to initial use?

I did not read the package insert when I received the bench but I ordered it in November 2020 to use in my shower and the finish is still good. So I did not use teak oil before use and I have not used teak oil on the bench.

Q: How many inches of clearance between the top and bottom shelf? Can it fit a 12" tall shampoo bottle?

I just went and measured mine. There is 13" clearance from the top and bottom shelf which - that should easily accommodate a 12" tall shampoo bottle.
