Q: Are these preassembled or you have to install the plugs?

These are preassembled extensions. They have a male pair on one end & a female pair on the other. I used them to add a needed 2 feet to a long speaker run.

Q: The image shows a male to male cable, however the description is male to female. Which is it?

There are two (2 wire) cables in the package. Each set has male plugs on one end and female on the other end. Each set has the wires connected in the middle, but they can be easily separated. If you do separate them you will have 4 wires, each having male on one end and female on the other. In fact that's what I did for my application. They are made with very good quality and have a snug fit.
