Q: Will work for a size 18 neck?

It is a little tight for my neck. I'm almost size 19. The 2 straps in the front will be a bit tight at first. But as you use it, you will get use to it. Plus the benefits of using this traction device outweigh the annoyance of the front straps.

Q: Is it ok to leave inflated for daily use instead of letting air out and refilling each time?

I leave it inflatable if I am going to use it hours aparts, but I take the air out when I won't use it on days end.

Q: Is the pump latex free?

It's black rubber, but I don't know if it contains latex. Actually mine is breaking apart...I need to find a replacement piece.

Q: Will this help one to sleep upright in airport etc? Will it support my neck?

Not sure. It might be too bulky for you to rest your head comfortably when sleeping in an airport. If you look at the picture, you can see that it holds your head up straight and the there might be too much space between your head and the seat. When you sleep your body slouches and your head might be too upright. I think you would be better off with a neck pillow that is designed for sleeping while traveling. This does support you neck for therapeutic use. It's a traction device and it works great for neck pain, kinks, stiffness cause by long periods of bad posture. I cut up a cotton t-shirt to wrap around my neck before putting this on to absorb any sweat caused by body heat.
