Q: why is it hard to flush the handle ?

Probably unrelated to the flapper itself. Check/adjust the chain length from the handle to the flapper (too short but long enough to let the flapper seal might make the handle seem hard to flush and too long might require excess travel in the handle, making flushing seem hard.

Q: Does this replace the Eljer 495-6055-00 flapper?

I used mine to replace a different flapper. The flapper you cited is for a different toilet. From one source on line it was about $4 cheaper than the 3030BP. I was unable to find any dimensional drawings on line so the bottom line is, I don't really know. Good luck.

Q: Will this flapper fit a 141-0777 Titan Eljer toilet? I cannot seem to pin this down for certain.

141-0777 ia one of Eljer's tanks used with several Titan models. Here is a link to the Eljer repair parts: http://www.eljer.com/assets/documents/eljer/parts/Parts_2498.pdf The Eljer flapper is PN 495-6077-00 and searching this comes up with hardware stores etc. selling the Lavelle 3030BP as replacement for the Eljer part.
